On Tuesday, February 19, 2013 at approximately 5:09 PM, Waterloo Police responded to Logan Avenue and Donald Street on report of an accident.

Officers found that a semi-tractor trailer, driven by Jimmy Parrish, age 39, of Russellville, Alabama was south bound on Logan Avenue when the traffic light turned to red. The driver tried to stop, blew his airhorn, but continued through the intersection. The semi collided with a car heading eastbound on Donald Street. That vehicle was driven by Thomas Butler, age 52, of Waterloo, IA. Butler was taken to Allen Hospital for his injuries.

A third vehicle, heading westbound on Donald Street collided with the semi. That vehicle was driven by Sabrina Lee, age 29, of Waterloo. She was taken to Allen Hospital for her injuries.

Two children, a 7 year-old male and a 5 year-old female, who were passengers in the Lee vehicle, were injured. The female was ejected from the vehicle. They were taken to Allen Hospital. All injuries are non-life threatening.

The driver of the semi was issued a citation for Failure to Obey a traffic control device.
