On Tuesday, November 15, 2011, at approximately 9:38 AM, Waterloo Police and Waterloo Fire Rescue responded to W.9th Street and Highway 218 southbound on report of a car and semi-truck accident. Officers found that the semi truck was southbound on Highway 218, ran the red light and was struck by the car as the car proceeded across West 9th onto northbound Highway 218.

The car driver, Tammi Cherrelle Fredericksen, age 41, of 3616 Laurinda Drive, Cedar Falls, IA was taken to Covenant Medical Center for numerous injuries and broken bones.

The driver of the semi, Michael Owen Butler, age 60, of 524 Sherman Street, Waterloo, IA was not injured. He was issued a ticket for a red light violation.

Fredericksen's car, a 1996 Chevrolet, sustained about $8,000.00 in damage. A trailer the semi was pulling sustained a broken axle. Southbound Highway 218 traffic had to be re-routed down Highway 63 to Highway 20 for several hours while the damaged vehicles and its contents were removed.

The Iowa State Patrol and the Iowa Department of Transportation assisted with the accident investigation and traffic control.
